JOB TITLE: In Home Nursing Respite Provider DEPARTMENT: HCBS REV: 01/16
SUPERVISED BY: Nurse Supervisor EMPLOYEES SUPERVISED: None
Must be at least 21 years of age.
Must be currently licensed to practice as a Licensed Practical Nurse in the State of Mississippi.
Must have a valid driver’s license.
Must have reliable transportation and meet applicable requirements for Millcreek’s Motor Vehicle Safety Program.
Must be willing to work flexible schedule to meet family’s needs.
Must complete the HCBS training and continued education program.
Must be a graduate of an accredited school of nursing with at least an associate’s degree.
PHYSICAL AND SENSORY REQUIREMENTS (with or without reasonable accommodation):
Walking; reaching; bending; pushing; pulling; lifting heavy loads (i.e., at least 25 pounds).
ESSENTIAL JOB FUNCTIONS:
Provide assistance with activities of daily living, i.e., eating, meal planning, bathing, dressing, hygiene/toileting, etc.
Provide assistance with light housekeeping needs that are directly related to health and well-being of the individual, i.e., bed-making, dusting, vacuuming, etc.
Follow appropriate protocol for completion of Serious Incident reports.
Prepare and administer medications and other medical procedures as ordered by a physician, accurately and in a timely manner.
Chart administration of medication and other medical procedures on the MAR and/or Nurses’ Notes, accurately and in a timely manner.
Accurately complete and submit Nurses’ Notes in a timely manner.
Adhere to safe transportation procedures at all times (emergency transportation of individuals only).
Work flexible schedule based on the family’s needs and to foster maximum use of authorized hours.
Attend trainings/in-services as requested and adhere to requests for personnel update information in a timely manner.
Adhere to Activity Plan and/or family instructions regarding personal care of the individual.
Complete an initial assessment of individuals receiving nursing respite services at admission.
Adhere to UltiPro usage, policies, procedures and timelines.
Keep supervisor abreast of pertinent information relevant to the provision of services.
Maintain a positive rapport with the individual and family.
Adhere to all Millcreek and HCBS MR/DD Waiver program policies and procedures.
Follows all safety policies and adheres to all worker's compensation program guidelines.
Other duties as assigned.
